Requesting Generate
REQUESTING GENERATE
To request the Generate application, go to the CIID website and follow the directions below:
Click on “Home” from the Menu.
Select “Generate” from the submenu.
Click in the “CLICK TO REQUEST” link on the right side of the page.
You will be taken to a new page with a form to fill out. Enter in the required fields.
