Requesting Generate

REQUESTING GENERATE

To request the Generate application, go to the CIID website and follow the directions below:

1. Click on “Home” from the Menu.

2. Select “Generate” from the submenu.

1. Click in the “CLICK TO REQUEST” link on the right side of the page.

1. You will be taken to a new page with a form to fill out. Enter in the required fields.

   1. Email

   2. Name

   3. State
